Changing GLP-1 providers: records, refills and the next clinical review

A transfer is more than canceling one subscription and opening another. Prepare the prescription history, product details, care handoff and remaining charges.

Collect the record before closing the account

Save the current prescription label, clinician contact, dispensing pharmacy, medication name, formulation, prescribed dose and most recent administration date. Include relevant clinical notes, recent requested labs and important side effects or prior treatment changes.

Unique Physique’s transfer policy illustrates the documentation a receiving practice may request: a verifiable prescription, label or prior-provider letter describing the current treatment. Other practices can have different requirements. Ask the new team for its list before arranging a transfer.

An incoming prescription still requires assessment

A new clinician does not have to continue the same medicine, formulation or dose simply because another service prescribed it. Supply the actual history and explain any treatment gap. Ask when the clinical decision happens and whether an upfront fee can be refunded if treatment is declined.

Do not minimize the gap to qualify for an established-dose plan. Lavender Sky’s published plans require documentation for some later tiers, and its refill rules distinguish a medication change from an ordinary refill. The paperwork should reflect the real treatment history.

Compare the incoming product, not only its ingredient name

The new supply may come from a different pharmacy or have a different route, concentration or added ingredient. Ask whether the proposed prescription is the same product, a clinically selected alternative or a compounded formulation with separate evidence limits.

If you receive a different compounded vial, obtain new measurement instructions from the pharmacist. An old number of syringe units can be wrong for the new concentration. Do not treat an administrative transfer as permission to calculate a dose yourself.

Plan the refill and the billing cancellation separately

Ask when the new practice can review you, when a prescription could be sent and whether the exact product is available. Pomegranate’s captured pricing page shows why a treatment page and an orderable supply are not the same thing. Keep an alternative continuity discussion open with the current clinician if the incoming service cannot fill the prescription.

Then address the old account’s future renewals and any processed order. Brightmeds and Zappy describe preparation or shipping cutoffs for refunds. Request written cancellation confirmation; opening a new account does not close the old one.

Leave the handoff with clear ownership

Confirm who handles symptoms, dose decisions, medication questions and refills after the transfer. Keep both the new clinician’s and pharmacy’s contacts. Ask how the old records will be shared and whether anything still needs to be requested.

If treatment is interrupted, get a prescriber’s restart instructions for the actual product. The safe endpoint is a reviewed prescription and a clear follow-up plan, alongside resolved billing; the endpoint is not simply a completed checkout.
